Documents to be submitted
  1. Copy of an account or Light Water
  2. Routing Medical Health System (SUS) and you have some documentation as RX, CT scans or medical report.
  3. Full address with postcode and a telephone message to the nearest residence.
  4. Minor patients, a copy of birth certificate and copy of ID, CIC of the father or mother. If an adult, a copy of RG, CIC
  5. Card SUS (Sistema Único de Saúde)

IMPORTANT

  1. On the day of consulting the patient spends the whole day in the hospital. Therefore, the patient should bring their home food , the Hospital does not offer lunch.
  2. If the patient is a minor, must be accompanied by parents.
  3. Failure to submit the documents requested above will result in the cancellation of the query.
  4. It is mandatory the presence of a companion of the family for patients over 60 years.
